Please arrive early to allow enough time to review your patient information and your current insurance coverage. We will also ask you to pay a portion of any past due balance and any current co-pays or deductible expense that would be due.

You MUST bring your latest Insurance card information EACH VISIT for updating our records. No exceptions. Without this information your appointment may be delayed or postponed.
NOTE: You may incur additional out-of-pocket expenses charges if you are out of network or self pay. This can be avoided if you provide updated insurance information before your appointment and check your coverage status.
Please be prepared that if your scheduled provider is out for illness or other reason we reserve the right to move your appointment to another available provider during the same time slot. We will attempt to make this a rarity but want to inform you of the possibility of this being necessary as we adjust for provider schedule variances. We appreciate your understanding.
